Client Data Database: This database stores all client names, geographic location(s), contact information (phone, fax, website, and email address), tax-ID#, business license, national provider identification number, medical billing claims address(es), office manager's name and contact information. This database also contains any state and federal accreditation certificates for the facility. Within the Client Database, there are six sub-databases: Human Resources sub-database houses the names of all the entity's employees, complete with nickname, social security numbers, dates of birth, start and termination dates, salary history, W-4's, I-9's, promotions, reprimands, benefits, worker's compensation claims, marital status, resident status (i.e. U.S. citizen), birthplace, Driver's license number, spoken languages, and emergency contact number(s), For each physician within the group, there is addition information fields in the human resources subsection, namely for the curriculum vitae, medical license number, National Provider ID#, all medical, state and federal licensure (i.e. state medical license(s), fluoroscopy license, and DEA license number), and hospital privileges. Contracting sub-database contains the entity's contracts with all health plans, complete with effective dates, termination dates, filing status (pending or complete), and a field to indicate whether the contract is physician specific or location specific. Credentialing sub-database lists all of the facilities where each physician is authorized to treat and evaluate patients, namely the name and location of the hospital or medical facility, complete with effective and termination dates. Also there is a filing status field that indicates whether the privileges are pending or complete. Medical Billing database contains Summary Reports featuring statistical analysis and error reporting for all medical claims generated by the entity; Fail reports featuring reports on all accounts and charges that failed with one or more errors for medical claims generated by the entity; and Pass Reports of all accounts and charges with no errors for medical claims generated by the entity. All three reporting categories are accompanied by the date and time the report(s) are ran, in addition to a listing of the scrub status (i.e. completed successfully or unsuccessfully). Information System Archive sub-database contains any expired record or document pertaining to the entity, and/or its employees. Reporting Module is a password-secured platform that allows each entity to retrieve any/all custom billing reports (i.e. total payments, charges and adjustments, along with daily daysheet totals). Compliance Database is a living database that stores all corporate organizational policies. Finance and Accounting Database stores entity's financial data, namely tax filings, monetary assets, profits and debits, and daily accounting ledger (i.e. daysheets, accounts receivables and account payables). Company Directory Databasestores the names of all employees, complete with birthdays, employment anniversaries, department locations, and phone extensions. Medical Billing Database is a living database containing Medicare rates, ICD-9 codes, CPT codes, fee schedules, and a glossary of medical/billing terminology. This database also contains a medical claims filter which scans all outgoing claims to ensure accuracy powered by sophisticated billing programming designed to screen and edit all medical claims. Time and Expense Tracking Database is a living database that contains and allows user to enter hourly rates for executive employees. There are fields to enter dates of service, hours of service, which clients services were rendered to, type of project/service and specific task, a comments box to cite notes pertaining to specific project, and a field to indicate whether or not fees for documented services are charges or waived. This databases interprets the information in the aforementioned fields to generate an invoice summary by employee that gives a detailed listing of all services dates for any work performed on a client's behalf and a monetary total for services rendered |
